Senator John Kerry surprised many with his win in Iowa. Pundits speculate that the general election will be won – or lost – on the topic of national security. However, this might be the first presidential election that the phrase “long-term care” has been brought up by candidates. What might that mean for long-term care (LTC)? A nationally known expert in long-term care planning, Darien, Illinois resident Matt McCann, has some ideas.

On January 13, current Democrat front-runner John Kerry released his “Compact with the Greatest Generation.” It has 4 key components:

1)  Assuring Quality Nursing Home Care;

2)  Protect Medicaid Support;

3)  Give Seniors More Options;

4)  Support Caregivers.

None of these components is likely to change the status quo of LTC planning. Middle-class and upper-middle-class Americans will still struggle to privately pay for the care that they want – in their own home. McCann says there is only one option that offers financial relief to citizens who want home care – and don't want to drain their life savings: LTC insurance. But less than 10% of seniors have policies. Sales of LTC insurance has increased dramatically for those in the preretirement years, 50 to 65. Still, many people don't plan and are in denial of the risk. Many seniors have waited too late and their health does not allow for the purchase of LTC insurance.

Component #3 (Give Seniors More Options) reads ” Allow Medicaid to pay for more alternative care outside of nursing homes to allow more Americans to age in their own homes.” This option already exists in some states – which have applied for and been granted waivers under the federal Medicaid program. On its face, less-expensive home care sounds like it makes sense. But for many years politicians and social scientists have warned of the “woodwork effect.” They speculate that if Medicaid covered home care, seniors would come “out of the woodwork” and apply for this benefit.

"The problem with Medicaid homecare is the quality of care. We already know that the quality of care in Medicaid nursing facilities is not the best. With state reimbursement rates going down, we expect this problem to continue. Homecare providers in the Medicaid system, when allowed, will not attract the best employees because of poor reimbursement rates. Plus, don't forget, Medicaid is still welfare and a person must spend down assets. For most people this is not something they want to go through. People work hard to accumulate assets and they don;t want to throw all that money away. In addition, most people want to protect the lifestyle of the healthy spouse. This means we must protect people's assets. LTC insurance does this," said McCann

What do John Edwards and Howard Dean say about LTC?

Edwards proposes to double federal support for respite care (respite care is temporary caregiving to allow primary caregivers a break – to take a vacation, for example). He also proposes to recruit and retain more LTC providers by treating them with respect and improving conditions at nursing homes.

Dean offers a 2 ½ page whitepaper on Long-term Care. Key components include: 1) give states flexibility to offer home care without a federal waiver; 2) invest modest federal dollars in home and community-based care; 3) to combat elder abuse, create a national registry of LTC workers; 4) require consumer protections for any LTC insurance policy to be eligible for a deduction; 5) earmark more National Institute of Health resources to study prevention and treatment of disabilities in our aging population.

Saying that no senior citizen should have to drain their life savings to get the long-term care they need, Senator Joe Lieberman has unveiled a plan to ease the burden on seniors and help their families afford reliable, high-quality long-term care.

Among other steps, Lieberman would give a tax deduction for private long-term care insurance premiums and create a new tax credit of up to $3,000 for family caregivers.

President Bush has supported the idea of private long-term care insurance since his first campaign. The President has proposed in his budget an above-the-line tax deduction for long-term care insurance.

McCann says that the high costs of long-term care along with the very high risk of any person needing care in their lifetime, combined with the ever aging population in the United States, makes it impossible for the federal government to pay for everyone's long-term care.

"The federal government has really made it clear since 1996 that people with assets need to plan with long-term care insurance. The only reason some people don't look into LTC insurance is denial. Even though 1 out of every 2 people who reach the age of 50 will need some form of care during their lifetime, some people will say it will be the other guy. As medical science continues to get better we all will continue to live longer. Things that used to kill us don't. This will continue the high risk of people needing some help before hey pass," McCann said.

After 20 rescue attempts, a hard-to-catch entangled California sea lion that was first spotted at San Francisco's famed PIER 39 docks, was successfully rescued, rehabilitated and released back to the ocean by The Marine Mammal Center in Sausalito, CA. Despite rainy skies, a crowd assembled at Rodeo Beach in the Marin Headlands, Sausalito, CA to watch as "Abagnale" made his way back to the ocean on February 5.

After nearly three weeks and 20 rescue attempts, an adult male California sea lion with a tight fishing line entanglement around his neck and mouth was released back to the ocean at Rodeo Beach north of San Francisco today by The Marine Mammal Center. Abagnale, as the sea lion was named after the 1960’s con artist Frank Abagnale Jr. who was always two steps ahead of authorities, was originally spotted at PIER 39 on the evening of January 1, and was rescued in Moss Landing Harbor by The Marine Mammal Center on January 24. With rescue options dwindling, and the animal’s life threatened, the team decided to try something never done before with wild, free-swimming marine mammals. Using darts, veterinarians injected a mild sedative into the animal to calm and slow him down in order to facilitate a successful and safe rescue. The Center’s rescue team, both in the water and on land, were then able to rescue the sea lion and transported him to the Center’s Sausalito hospital headquarters.

At the Center, veterinarians anesthetized the malnourished sea lion and removed the monofilament nylon fishing line that was deeply imbedded into his neck and mouth, performed a physical, and they administered antibiotics and pain killers. During his 13-day-stay at the hospital, Abagnale ate just over 100 pounds of herring. On February 2, veterinarians examined the pinniped again and were pleased to see that the wounds were beginning to heal and that there appeared to be no other complications. Given his regained strength and healthy appetite, veterinarians feel confident that Abagnale will survive quite well in the ocean and that the saltwater will help his wounds continue to heal.

Abagnale’s entanglement story, sadly, is a common one seen at the Center. In 2009, approximately eight-percent of the 1,702 marine mammals that volunteers and staff at The Marine Mammal Center rescued stranded as a result of entanglement in marine debris. The Center responds to seals, sea lions and other marine mammals that are in need of rescue along 600 miles of coast between Mendocino and San Luis Obispo Counties and through 800 volunteers and a small staff, provides medical care to each animal admitted with the goal of getting those patients healthy enough to be released back to the ocean.

“What’s maddening is that you look at the wide array of reasons why marine mammals strand such as illnesses and malnourishment and this one – marine debris – is something we can control if we just change our behaviors and attitudes about how we discard plastics, fishing line and other trash that can become marine debris,” said Jeff Boehm, executive director at The Marine Mammal Center. “We hope people will hear Abagnale’s story and make a pledge to eliminate marine debris, and in turn, help marine mammals and the ocean.”

In December 2005, the Center came to the rescue of a 50-foot-long humpback whale in the Pacific Ocean near the Farallon Islands that was entangled in more than 40 crab-pot lines. In 2008, Dr. Frances Gulland, director of veterinary science at The Marine Mammal Center, along with a team of researchers, examined a 51-foot-long sperm whale that had died and washed up on a California beach – a victim of marine debris. Dr. Gulland and the team discovered nearly 450 pounds of commercial fishing nets, braided rope, and other debris inside the animal’s stomach – a pile the size of three large refrigerators.

The Center’s water rescue team is a specialized unit of 14 trained volunteers who perform water and dock or pier-based rescues of marine mammals, the majority of which are entangled in marine debris such as plastics, netting, fishing line and crab-pot lines, among other things. Disabled 80-year-old Terre Haute woman will "live again" with her new scooter. After a 20-year battle with heart disease, Dorothy Adams was ready to give up and accept a life of physical inactivity. Mrs. Adams, 80, said a cardiologist has tried "everything known to man" to unclog her arteries, but nothing has completely done the job. "I can't walk a quarter of a block without chest pains," Mrs. Adams said. "Going from being active to being stuck in a wheelchair, that's been the hardest part. Everything else I can handle, but losing my freedom has been the toughest."

She depends on her husband, Bob, to push her around their assisted-living facility in a manual wheelchair. His arthritis is getting progressively worse, though. "We looked at the future and decided we needed to do something right away," Mrs. Adams said.

Thus, untold millions of anesthetics occur annually throughout Africa's 54 countries with inadequate drugs, and equipment and untrained personnel.

This September The Kenya Society of Anesthesiologists is taking the lead toward solving this problem as host to the 4th All African Anaesthesia Congress in Nairobi, Kenya. The All African Anaesthesia Congress only takes place once every four years and this year they are getting the assistance of partners from Johns Hopkins Hospital, Vanderbilt Hospital, The Society of Critical Care Medicine and a number of other academic medical institutions within the United States.
The Johns Hopkins Center for Clinical Global Health Education facilitated telemedicine presentations for education team members who are currently not be able to travel to Kenya such as Dr. Benjamin Carson (Presidential Medal of Freedom) and renowned critical care physician Dr. Peter Pronovost (2008 Time Magazine's 100 most influential people in the world).

The mentoring and introduction of students to the status of anesthesia in Africa is important in producing leaders who will continue to address this significant issue. After a stiff competition between an academically competitive field of medical students, Doctors for United Medical Missions, Inc. (DrUMM) has granted Masters of Public Health candidate Lee Goeddel with a Dean H. T. Sampson Travel Scholarship. This travel scholarship will provide Lee with the opportunity to be a part of an international effort to be a part of an international effort to improve health care opportunities for people living on the African continent.

The scholarship is named after the late Dean Henry Thomas Sampson of Jackson State University, whose relentless devotion to education has been acknowledged by the naming of the Jackson State University library in his honor.

Lee Goeddel will be traveling to Kenya with a team of physicians from the Johns Hopkins Hospital/Society of Critical Care Medicine Education team. It is expected that as a result of this exposure Lee will be inspired to utilize his emerging public health skills to bring awareness and solutions to the unnecessary deaths that occur during and after operative care in Africa.
