Nicotine Gum Maker's Concern Raises Concerns

The maker of Nicorette gum - GlaxoSmithKline (GSK) - issued a May 3rd press release in response to a May 2nd N. Y. Times article entitled "A Quitter's Dilemma: Hooked on the Cure." The Times piece quotes a GSK consultant as saying, "We estimate 36.6 percent of current gum users are engaged in persistent use." Concerns over a rising tide of chemically captive customers may be secondary to a more fundamental concern indirectly raised by the GSK press release: do "any" quitters using nicotine gum as a stand-alone tool actually break free from nicotine?

Last year two GSK consultants combined and averaged all seven over-the-counter nicotine patch and gum studies. The shocking results were published in the March 2003 edition of Tobacco Control. They found that only 7% were successful in quitting smoking for six months. Amazingly, GSKÂ’s May 3rd press release includes the admission that among those who quit smoking, "7% used the gum for 6 months or more." Correct me if wrong but isn't seven minus seven still zero?

Upon discovering how to abstract pure nicotine from the tobacco plant the pharmaceutical industry actually rewrote the definition for quitting. Prior to that moment all methods of quitting included breaking free from nicotine too, the sine qua non of smoking.

The modern quitter now has four important considerations in comparing and picking cessation products, procedures or programs: the smoking cessation rate, the nicotine cessation rate, the length of time they quit (for purposes of making accurate comparisons), and how results were verified.

As for verifying results, self reporting relies entirely upon the honor system, expired carbon monoxide levels test for smoking but do not test for the presence of nicotine, and blood cotinine testing (a long-lasting nicotine metabolite) is the common method for detecting recent nicotine use.

Few NRT studies examine cotinine levels but for those concerned about continuing dependency upon nicotine it can be a rather important piece of information. In a 2002 nicotine lozenge study by GSK consultants, victory in quitting smoking was declared at six weeks while study participants were offered up to 20 lozenges a day for six full months.

GSK's press release also asserts that NRT "doubles a smoker's chances of quitting versus cold turkey." We now have four large real-world quitting surveys in which performance of smokers quitting cold turkey is directly compared against those quitting with NRT -- California, Minnesota, London and Quebec. In each case cold turkey quitters performed as well as NRT quitters at six months - zero advantage.

Not only has the pharmaceutical industry redefined quitting, amazingly, it has taken the liberty of redefining who is and isn't a cold turkey quitter. It's fully aware that "real" cold turkey quitters - those actually motivated to quit cold turkey - were not invited to compete in formal NRT studies.

In almost all NRT studies, participants were recruited by being promised a 50/50 chance of receiving weeks or months of free NRT products. When assigned to the placebo group instead, a significant percentage were somehow able to sense that the flow of nicotine to their brain had ended.

In one of the seven OTC NRT studies - Sonderskov 1997 - at study's end only 18.3% of those in the placebo patch group believed that they had received the real nicotine patch. The authors openly admit that "the effect of such a blinding failure would probably be a reduction of the placebo effect."

Chance? Coincidence? Apparently not. A new study in the June 2004 edition of Addictive Behaviors identified 17 NRT studies which assessed blindness. The authors assert that "12 studies found that subjects accurately judged treatment assignment at a rate significantly above chance."

Has a billion dollar industry been built entirely upon meaningless odds ratio victories that reflect little more than high placebo group dropout rates due to seriously frustrated expectations? If so, at what cost in terms of quitting opportunities, time, and lives needlessly lost? At what cost in terms of the integrity of science?

“We truly regret having to resolve this matter in court,” said EEF President Richard Bates. “But ESS’s actions have left us no choice but to seek redress through legal action. “We really do business the “old fashion way” said Bates, “When we contract with a company we live up to our obligations, and then expect to be paid following its completion. Like any going concern, this is critical to keeping our doors open, expanding our offerings and continuing our vital mission. ESS is clearly in breach of the financial aspects of our contract and despite numerous attempts to settle this amicably, in the end we were left with no other option other than this legal action.”

The EEF lawsuit stems from the organizations agreement to validate training classes ESS offered on Indoor Air Quality Management, and assuming these sessions yield the appropriate body of information, provide ‘registrations,’ a form of membership classification respected by various government agencies that transact with the Foundation and acknowledge its considerable expertise in the environmental field. Gaining a registration from EEF in effect recognizes the awarded as possessing the necessary degree of knowledge in a specific environmental arena.

The Foundation completed all the stipulated services according to its contract with ESS and has yet to be paid after nearly a year since fulfillment of the project. Two other organizations ESS enlisted to provide some credibility to their company, The World Safety Organization and InfoChem, have just settled similar disputes in out of court settlements.

“We’ve been providing education services since the 1980’s without ever before having to resort to legal action to satisfy an outstanding bill,” Bates emphasized. “This is certainly an unwanted event in our history. We’re here to educate people, companies and government agencies about steps they can and in many instances must take to protect the environment and remain in compliance as well as responsible citizens. We conduct seminars, gather and distribute critical educational information about the environment; we establish programs to help every day individuals and companies--all towards our crucial goals. We live on donations, contributions and a few revenue raising services. We simply must resolve this issue and get on with our work.”

The first round of judging for the contest to design the next X Prize closes Friday (November 14, 2008).

The renewable sector currently employs 116,000 people and generates $19 billion dollars in annual revenues.

During the video, Beitel explains how retirement plan investment could play a major role in increasing renewable energy production.

The renewable energy sector is also expected to experience rapid expansion under President Elect Barack Obama who backs federal investment in renewable energy, along with regulatory and tax support for clean electricity and alternative fuels. The Obama administration plans to invest $15 billion annually in renewable energy, and sees the industry as a key source of new American jobs. This year in Colorado, Vestas, one of the world's largest wind turbine manufactures, has opened 2 new manufacturing plants that will employ more than 1,300 people.

Beitel acknowledges the tough economic times: "While $10 billion dollars sounds like a lot of retirement dollars to invest in renewable energy, in reality it is only about 1/3 of 1% of the roughly $2.7 trillion dollars in US 401(k) plans."

The proposed "Invest Renewable" X Prize will encourage Fortune 500 and smaller companies in the US and Canada to add a renewable energy sector investment option, such as the Calvert, Guinness Atkinson or Firsthand alternative energy fund to retirement plans. The first company or team of companies to generate $10 billion in renewable energy investment through their corporate retirement plans will split the $10 million X Prize amongst employees who have chosen to invest in renewables.

With the UK's ageing population making the headlines, research shows that the 'Bank of Grandma and Grandpa' is alive and well with British grandparents spending on average nearly a quarter (22%) of their income on their grandchildren*.

Far from being dependent on their own children, the research by Experian highlights that grandparents are amongst the most financially literate in the UK and ready to share the benefit of their knowledge and experience with their loved ones. A staggering 83% of grandparents understand the importance of a healthy credit history (http://www. creditexpert. co. uk/credit-history. aspx ) on their finances and 41% rate their own financial literacy as 'high'. Conversely, nearly a quarter of 25-34 year olds (23%) and over a third (33%) of 35-44 year olds described their financial literacy as 'low'**.

Whilst 80% of grandparents revealed that they own a credit card, nearly half (42%) claimed to have no debt on their credit cards due to good personal finance management, with over two thirds (69%) stating that they always pay their bills on time to avoid late notices or penalties***.

There are currently nearly 850 co-sponsored WSU/DMC residents in over 65 specialty and subspecialty departments.

Fearing a closure of their medical residency training programs and an impending health care crisis for Metropolitan Detroit, residents jointly sponsored by Wayne State University and the Detroit Medical Center are holding a rally in support of a contract agreement on Friday, October 13. There are currently nearly 850 co-sponsored WSU/DMC residents in over 65 specialty and subspecialty departments.

The rally will take place on Friday, October 13 between 12:00-1:00 pm in the grounds between the DMC’s Detroit Receiving Hospital and Scott Hall of the WSU School of Medicine.

The partnership between the Detroit Medical Center and Wayne State University has been functioning under successive contract extensions since 2001. Ongoing contract disputes have come under close scrutiny of the Accreditation Council for Graduate Medical Education (ACGME) after the failure of previous negotiations resulted in the closure of one of the nation’s largest orthopedic surgery residency programs at the end of the last academic year.

The shut down of all jointly sponsored specialty and subspecialty training and medical care provided by WSU/DMC residents could result from either the failure of a contract agreement, or ACGME decision that the institutional support is no longer conducive to the educational and clinical requirements for accreditation. The latter may be determined by the ACGME during a WSU/DMC institutional review scheduled for November. The ACGME has experience in mass relocation of resident physicians after the transfer of residents necessitated by the Hurricane Katrina disaster.

Residents believe that termination of the WSU/DMC partnership would harm the individual goals and missions of both institutions. They also fear that their relocation would cause a health care crisis in the region.

In a September correspondence to the residents by President of the WSU/DMC Resident Council, Paul Bozyk, M. D., wrote, “We are dedicated to the service of medical care-giving irrespective of race, religion, gender, age, or socioeconomic status. We acknowledge our vital role in the provision of care for the most vulnerable members of our community. As the City of Detroit is already medically underserved, and comprised of the largest number of uninsured persons in the state, we have great fear that the loss of physician workforce at such a grand scale would have dire consequences to a community with the greatest need. Such an occurrence would have financial and healthcare repercussions that would affect all regional healthcare providers and cause immeasurable damage to the City of Detroit and its surrounding areas.”

WSU/DMC residents have organized a rally to show support for continued partnership between WSU and the DMC and encourage immediate vigorous negotiations as a formal review by the ACGME in November rapidly approaches. This rally is supported by the WSU School of Medicine Student Senate, comprised of student leadership representing over 1,000 medical students.

The U. S. Federal Trade Commission (FTC) has determined that fibers made from PTT, the latest polymer innovation from DuPont, offer a unique combination of attributes which merit a new generic name, triexta. PTT polymer sold for the manufacture of triexta for apparel and flooring applications is sold under the DuPont™ Sorona® brand.

Triexta fibers provide a unique combination of exceptional durability and stain resistance compared to previously available fibers. The FTC based its decision on testing of residential carpets, which demonstrated that triexta significantly outperforms carpets made from polyester in resilience and durability.

Today's announcement is the latest development in the long tradition of DuPont innovation in materials science. DuPont shaped the polymer and fiber industry with the discovery and commercialization of well-known high performance fibers such as nylon, rayon, DuPont™ Kevlar® and Nomex®.

"The designation of the generic triexta is significant and further validates the importance of the superior performance and distinct attributes offered by Sorona®," said John P. Ranieri, vice president and general manager -- DuPont BioMaterials Group. "In addition to the performance advantages recognized by the FTC, Sorona® is made in part from renewably sourced ingredients. It is the most transformative polymer to be introduced into the fibers industry in the last 20 years."

DuPont began production and distribution of Sorona® through its global network in 2000. Fabrics made with Sorona® offer a unique combination of attributes that are perfect for today's busy lifestyles, including: exceptional durability and softness; built-in permanent stain and ultraviolet (UV) ray resistance and outstanding elasticity while resisting stretch and sag over time. Sorona® fabrics provide a carbon dioxide reduction of 30 percent while the Sorona® manufacturing process reduces greenhouse gas emissions by 63 percent, compared to conventional nylon made from petroleum.

Fibers made from Sorona® are sold into a variety of markets including floorcovering, apparel and upholstery.  A recent article in the Time Magazine (11/12/01) on "Who Should Get Flu Shots?" has, to its left, another item under "Good News -- Say Yes To Stress." The comment of interest relative to the article and today's difficult times when news reports can produce concern for public safety as well is, "...stress you can't do anything about...diminishes immunoglobulin production and weakens the immune system." Certainly the current global surge of SARS (severe acute respiratory syndrome) would be relevant here.

 As a retired clinical psychologist and psychoanalyst, I have been examining ways we may empower ourselves and better face what is before us in these difficult times. With so much uncertainty in our lives, with nagging new information bombarding us day after day that is clearly anxiety arousing, many may turn to any of a wide variety of relaxation techniques such as exercise, meditation, massage, biofeedback or other alternative therapies. There may be an interest, for some, in seeking professional psychological help for what may be akin to a post traumatic stress disorder. Many have been turning to a religious focus in the current time of stress and crisis with widely reported increases in congregation attendance.

 As still another significant alternative open to us, perhaps the Human Immune System could be considered our strongest defense of all. Researchers have agreed, for example, that the 40% who survived the Bubonic Plague, did so because they had stronger, more balanced Immune Systems. Today there is much we can do to create a safety valve program, maintaining and balancing our own immune systems, thereby giving us the best chance of all to deal personally with whatever potential threats or stressors we may confront in our lives (such as the current SARS).

 Certainly diet and the adage, “You are what you eat,” is a major consideration. Many foods including any containing preservatives or color dyes, high sugar content or sugar substitutes, or many other unhealthy additives may compromise the immune system (remember Mrs. Gooch who almost died from consuming a popular soda and, thanks to her savvy father, discovered the cause of her illness in time? The Sales Lead Management Association (SLMA) yesterday released the list of winners of the 50 Most Influential People in Sales Lead Management in 2010. Included on the list are two executives of PointClear, the Atlanta-based prospect development company.
