Continuity of Care Record (CCR)

The Continuity of Care Record (CCR) standard is an international standard designed to organize and make transportable a set of basic information about a patient's health care in a manner that is accessible to both clinicians and patients. Developed by American Society for Testing and Materials International (ASTM), one of the largest voluntary standards development organizations in the world, the CCR incorporates the most relevant administrative, demographic, and clinical information facts about a patient's health.

According to Dr. David Kibbe, Director for the Center for Health Information Technology of the American Academy of Family Physicians (AAFP), “… using the CCR standard could create basic interoperability among PHRs and EHRs and other HIS systems across the country in a very short time, leading to greater convenience for patients and consumers who would no longer have to hand-write administrative and clinical information into the ‘clipboard’ at every new doctor's or hospital visit.”

Commons eXchange Protocol (CXP)

The Commons eXchange Protocol (CXP) is an Internet-based public domain transfer protocol for connecting any patient to any health care provider.

David Cameron’s Adviser of Health and Safety Law and Practice, Lord Young has published a report of recommendations with the aim to remove the fear of litigation that apparently exists unnecessarily in businesses today.

The report, titled “Common Sense, Common Safety” was published on the 15th of October and in brief contains suggestions such as introducing the recommendations in Lord Justice Jackson’s review of civil litigation costs.

Michael Jefferies, Managing Partner of First Personal Injury, believes that Lord Young’s suggestions will restrict access to justice, on the basis that the proposed changes to the system would not favour the personal injury firms and effectively put them out of business, reducing the likelihood that accident victims will gain the compensation they are entitled to.

Currently lawyer success fees are paid by the losing party, but Lord Justice Jackson has recommended that these are instead paid by the client, through increasing the client damages by 10%.

However Michael Jefferies believes that “if the solicitors’ success fee is taken from the client, then the amounts received by clients will be even lower than they receive now. If they don’t actually receive their full compensation then they may run out of money to pay for the nursing care or have to reduce their standard of living. The whole point of the law of negligence is that the negligent party, through their insurers, puts the client back in the position that they would have been had the negligence not occurred.”

“Whilst the poor clients are put in such a terrible financial situation, the insurers’ shareholders will be enjoying a windfall as a result of paying out less.”

In relation to Lord Justice Jackson’s report, published in January 2010, Lord Young also welcomes the recommendation of fixed fees for injury claims under £10,000 (with the possibility of this even being raised to £25,000). The suggestion of fixed fees causes concern for Michael Jefferies who feels that this could put personal injury firms out of business as well as extending the length of time it will take to close a case.

“Defendant insurers control the disclosure of vital documentation and can therefore offer to settle a case at anytime. Meanwhile the only weapon the claimant solicitor has is the costs there will be to the defendant if the case is forced to go to court because they are not getting on and settling the case properly.”

“The defendants ability to string cases out thereby delaying the clients getting their rightful compensation, will make it uneconomic for claimant lawyers to conduct the work as the fee will be fixed no matter how much the defendant insurers string the case out. It is therefore likely that they will stop doing such work, thereby reducing innocently injured claimants finding access to justice.”

74 per cent consider their overall performance as a leader to be better than that of other executives in their industry 89 per cent say they are doing better than others when it comes to "willingness to collaborate with others on their team" 79 per cent say they are doing better than others when it comes to demonstrating "awareness of the issues and challenges facing their employees" and 64 per cent consider their "commitment to their own leadership development" to be better than other executives.

These are the findings of an Ipsos Reid survey conducted on behalf of The Refinery Leadership Partners from January 3 to February 22, 2008. For the survey, 151 telephone interviews of 67 Canadian and 84 American senior-level business executives were conducted. Robotic Surgical Technology at Cedars-Sinai Medical Center allows surgeons to see inside the mitral valve, gain a better understanding of its pathology as compared to traditional sternography, and perform less invasive surgery for Mitral Valve Repair.

Certified public accountant (CPA) Steve Garelick of West Hills, CA, knew the complex heart surgery (http://www. cedars-sinai. edu/8528.html) he needed in late February to repair his leaking heart valve couldn't be postponed until after tax season. Yet thanks to robotic surgical technology available at Cedars-Sinai Medical Center (http://www. cedars-sinai. edu/), Garelick, 54, was back in the office on a part-time basis just two-and-a-half weeks after his surgery.

Garelick's surgeon, Alfredo Trento, M. D., (http://www. cedars-sinai. edu/2507.html) director of the Division of Cardiothoracic Surgery (http://www. cedars-sinai. edu/2365.html) at Cedars-Sinai, has performed 70 robotic mitral valve repair surgeries over the past two years, a surgical volume that ranks fourth in the U. S. among cardiovascular surgeons who perform this highly skilled surgery.

"With robotic technology, surgeons can visualize the mitral valve in such a way that they gain a much better understanding of the valve's pathology when compared to traditional sternography. We can actually see inside the valve … it's amazing technology," said Trento.

Cedars-Sinai is the largest mitral valve repair (http://www. cedars-sinai. edu/2360.html) center in the Los Angeles area, completing approximately 100 repairs of different types annually. By repairing the valve rather than replacing it, a patient can avoid long-term use of blood thinners and problems such as infection.

The mitral valve opens and closes, controlling the blood flowing into the left side of the heart. When it becomes damaged, it can't completely seal the heart's left ventricle which causes the heart work to harder and leads to potential complications like congestive heart failure.

Mitral valve repair involves delicate reconstruction of valve tissues. Traditional repair (sternotomy) is performed by sawing open the breastbone and spreading the ribs apart to expose the heart. The less invasive procedure (http://www. cedars-sinai. edu/2359.html), which requires only a few very small incisions under the right side of the breast, uses Intuitive Surgical's daVinci Surgical System (registered trademark).

From a console a few feet away from the patient, the surgeon views a high resolution 3D image of the surgical site through a camera. Miniaturized instruments -- inserted into the body through a series of dime-sized incisions -- are controlled by the surgeon's hand movements which are scaled, filtered and translated into precise movements completed by four robotic arms located next to the operating table. The system relays force feedback sensations back to the surgeon that provide a substitute for tactile sensation.

In addition to improved visualization, Trento said the robotic procedure has less risk of infection, less blood loss and faster post-hospital recovery than traditional surgery.

Sternotomy typically takes between one and three months post-surgical recovery time.

"If I decide that I can repair a patient's mitral valve rather than replace it, I'll do the procedure robotically."

Garelick was referred for the procedure after he began experiencing fatigue following activities such as swimming and gardening. According to his doctor, Ilan Kedan, M. D., a cardiologist with the Cedars-Sinai Medical Group (http://www. csmc. edu/1229.html), Garelick was an excellent candidate for the da Vinci procedure because he was not overweight and didn't have any coronary artery disease nor other complicating illness.

"I had always considered myself to be very active," says Garelick. "About six months before the surgery I'd find I was winded and have to rest after swimming only 100 yards. I started to cut back on exercise, thinking it was just normal aging. It turned out it wasn't. I thought I was healthy..finding that I had mitral valve prolapse was a real surprise to me."

Garelick said he wasn't concerned about having a robot as part of his surgical team. "I'm very grateful to Drs. Kedan and Trento for their knowledge and foresight in this field."

The Institute of Groundsmanship (IOG), the leading membership organisation for grounds professionals involved in the management of sports pitches, landscape and amenity facilities, has announced a major ten-point strategic ‘Challenging Perceptions’ development plan that will carry both the IOG and the grounds care profession it represents forward in unprecedented style for the next six years.

With ‘quality surfaces supported by quality services’ as the central thread, the strategic areas highlighted by the IOG for development also include:

Geoff Webb comments: “The Challenging Perceptions strategy lays the foundations for a new era of the IOG as the Institute and its members continue to establish wider awareness of their role as the preparations for the Olympic Games gather pace.

Characterised by “modern and forward thinking”, the groundbreaking strategic signposts in Challenging Perceptions have received industry-wide applaud, with organisations such as the Central Council of Physical Recreation, Lawn Tennis Association, Skills Active, Sport England, The Football Association and the Waste and Resources Action Programme all endorsing the planned developments.

Indeed, says Minister for Sport Richard Caborn: “IOG members provide an important service to sport…delivering high quality playing surfaces…The recommendations outlined provide a firm foundation for success and I welcome them.”

The IOG has highlighted a number of factors for immediate action within the 10 strategic themes of Challenging Perceptions, and these include:

Improved leadership and good governance – continue the review of IOG internal processes and systems to ensure best practice, efficiency and effectiveness; and complete the appointment of a dedicated executive team to deliver the strategy.

Improved working partnerships – engage and work closely/better with: Government departments such as the Department for Media, Culture and Sport, the Department for Education and Skills, and the Department for Trade and Industry; sector training and skills councils including LANTRA and Skills Active; other industry professional organisations such as API, BALI, BIGGA, GTC, LDCA, NPFA and SAPCA, as well as the newly-established Institute of Sport, Parks and Leisure (ISPAL); plus local government and organisations that employ grounds and amenity professionals.

There is also a need to build on the already important relationships with the National Governing Bodies of Sport for bowls, cricket, football, horse racing, rugby league and rugby union.

Quality surfaces supported by quality services – in the build up to the 2012 Olympic and Paralympic Games, the IOG is determined to work tirelessly to raise the profile of ‘quality surfaces supported by quality services’.

The IOG will also work with industry partners to raise the profile of correct technical standards and construction methods, as well as campaign to ensure that the correct maintenance and remedial regimes are applied to exiting facilities.

Also on the agenda is the development of a comprehensive network of expert quality advisors.

Professional services, membership and communications – a continuous programme of improving the image and value of the grounds professional; and by early 2007, the IOG will be setting further targets to support its successful drive to continue to raise the IOG’s core membership, particularly with regard to new and younger entrants to the profession.

Improve communication with both the IOG membership and the wider industry – the latter, for instance, by highlighting industry issues in an imaginative and innovative way to national and regional media.

Training and education – the new Level 2 National Certificate in Sports and Amenity Turf Maintenance, due to be introduced by the end of 2006, will be complemented by a new Advanced National Certificate in 2007.

Further exploration of the opportunities for the delivery of IOG-endorsed courses through online learning and visual learning packs; investigate opportunities available to reach volunteer grounds professionals; and, by autumn 2007, the instigation of a new network of courses that reflect customer demand geographically.

In addition, active participation in the relevant sector Skills Councils will ensure that grounds management is recognised as a skilled and supported profession.

Health and safety – adopt a best practice stance on health and safety by strengthening the current services provided by the IOG health and safety advisor; by making health and safety and risk assessment a key feature of training and education; and by disseminating practical information in an easy-to-understand way.

Also, the IOG will work with partners to bring expert influence to bear on proposed standards.

Research and development - using the IOG 2000 Fund as the benchmark for pioneering research, the new ‘2012 Research and Development Fund’ will be targeted at projects that have the potential to benefit individual grounds managers, the wider industry and lead to better quality playing surfaces.

With partners, the aim is to invest at least £500,000 over the period 2007-2012, including partner contributions.

Continuing the work of the IOG 2000 Fund - which was originally established as a Millennium Programme to make grants available to individuals and organisations for education, research and benevolent purposes into the development and provision of quality surfaces – the 2012 Research and Development Fund will have a wider remit and include:

A proactive approach in identifying and determining key industry research areas Extending the range of involved partners Improving the explanation of the research as it is undertaken (interim reports) and practical outcomes.

Natural environment - adopt best practice on all issues relating to the natural environment, by engaging more proactively with environmental agencies and organisations, and focusing on, for example, legislative compliance, water, recycling and disposal of waste products, and the use of and disposal of chemicals. Also, improve the dissemination of that information via the IOG website, publications, courses, events and exhibitions.

Events and exhibitions – the IOG is committed to an energetic programme of events and exhibitions; both IOG SALTEX and IOG Scotsturf will continue as annual events; and the IOG will develop a considered strategy of regional events that will continue the Challenging Perceptions’ ten strategic themes.

2012 opportunities – the IOG will actively seek to engage with the Olympic and Paralympic organising authorities to develop opportunities that arise as the UK approaches 2012, and where possible will take the lead in specifying, advising on and monitoring the technical standards required, using these facilities as showcases for best practice groundsmanship.

In addition, the IOG will campaign to ensure that the post-Games natural turf and synthetic turf legacy is delivered on a sustainable basis.

Geoff Webb concludes: “In addition to also instigating the industry’s first ever accurate survey of the actual size of the UK’s grounds care industry, the strategy highlights the areas where the IOG feels that it both should and could make the most impact over the coming years.

KAUST will recruit leading scientific researchers, including faculty, administrators and students, on an international basis. Following that appointment, recruitment of other top administrative positions will proceed.

KAUST's academic model is structured around four Research Institutes, each of which will include Research Centers devoted to a particular set of issues or challenges that could benefit from concentrated scientific and technological study. The Institutes include among others:
The Resources, Energy and Environment Institute, with Centers initially devoted to challenges in energy research; water and sustainable development. Biosciences and Engineering Institute, with Centers focused on environmental bioscience, industrial biotechnology, agricultural biotechnology, health science and technology. Materials Science and Engineering Institute, with Centers interested in problems relating to polymers, membranes and nanomaterials, catalytic chemistry and materials for high-stress environments. Applied Mathematics and Computer Science Institute, with Centers focused on language software technologies and computational linguistics, IT innovation.

"The animating principle of KAUST is to organize interdisciplinary teams around specific scientific and technological problems in order to innovate and achieve breakthroughs," said Minister Al-Naimi. "This is what every leading academic and research expert tells us is the key to the future of innovation. And they're all watching KAUST with great hope and curiosity."

KAUST's research will be carried out at its campus in Saudi Arabia, as well as at many other leading research institutions through a dynamic global grants program that will begin this year. The preliminary budget of the Global Research Partnership grants could reach up to $1 billion over a period of 10 years. It will be awarded on a competitive basis to other collaborative institutions and researchers in the areas of advanced concepts in desalination and water supply; carbon capture and hydrogen-rich fuels; industrial biotechnology; catalysis and polymer chemistry; language software technologies and computational linguistics; and scientific computing.

The competitive Scholarship will cover the costs of tuition, and room and board at the recipient's university or college. Upon graduation, recipients will continue their graduate studies at KAUST.

Prior to that, Michael was Senior Marketing Manager responsible for direct-to-consumer efforts with Amevive® at Biogen Idec, Inc. and Remicade® Product Manager at Centocor, Inc. His previous experience also includes ten years in speciality sales with Amgen, Inc and Fujisawa Pharmaceuticals, Inc.

The Fitness Center
Learning that not everyone wants to go to the typical weight pounding gym, Jennifer decided to take the environment of Innergy Studio and put that into a fitness center. You will not find weights slamming and grunting at this fitness center.

October 1, 2007 marked the first day open at Innergy Strength and Cardio, Inc. Cool, calming colors on the walls, and vibrant upholstry colors on the machines make this fitness center so appealing to both men and women.

"Bringing what has worked at my studio, and combining that with the fitness center concept, I have created an environment that is not intimidating or over-stimulating," states Jennifer. "From the moment you walk in the door, you will feel at ease, and comfortable."

Choosing the types of machines to offer at the fitness center is in the name: strength machines includng a variety of machines to work the whole body individually. Cardio machines include treadmills, ellipticals and bikes. There is an area to stretch out and get in that much needed core work and dumbbell strength training as well. Members are allowed full access to the center with their access control cards everyday from 6:00 a. m. - 9:00 p. m. Staff hours are after 5:00 p. m. daily. The DVD
Jennifer has always aspired to produce a Pilates Video. "I can honestly say that I have not found a true beginners pilates video out there. Most beginner videos that I have seen show movements that are not suitable for beginners. My vision with this video is to give those wanting to learn pilates a true beginner fundamental workout that most can comfortably perform in the comfort of their own home."

Within the video, there are 4 people to watch. The viewer simply chooses their fitness level and watches that person. The background music is calm and relaxing. The video is sure to give viewers the knowledge to learn the precise pilates fundamental movements at their own pace.
