Cardiovascular disease is the Number One killer in America, claiming 2,400 lives each day. C., a Virginia-based cardiologist and a spokesperson for the alliance. "We are launching the Guarding Hearts Alliance to make sure that America's heart patients are heard in Washington. We support the intention of recently proposed Medicare regulations, but we cannot stand by as proposals are adopted that will hurt patients and their access to the highest quality cardiac care possible. Our focus is on our patients and protecting the progress we've made in fighting heart disease."

Potential Impact of Medicare and Health Care Reform Proposals
America's cardiologists have made great advancements against heart disease, reducing heart-related deaths and the severity of heart-related illness by 27 percent during the past 10 years. The urgent need for this specialty will continue to grow by 60 percent in the next 15 years as baby boomers age.

However, proposed changes to Medicare and certain provisions in legislation before Congress threaten the advances in cardiac care that have benefited patients across the country. Guarding Hearts Alliance is fighting to ensure that patients' safety comes first, ahead of well-intentioned but ultimately harmful changes that would reduce access to the best cardiac care possible.

Today, there are Medicare proposals that, if enacted, will have life-threatening impact on cardiac care in America. These proposed changes could:
 Limit access to in-office imaging equipment, which would delay diagnosis and add another layer between doctor and patient.  Force cardiologists to lay off staff, close outlying offices, reduce services and limit the number of Medicare patients they treat.  Increase health care expenses, both to the system at large from the use of more expensive testing locations and to patients, via increased patient out-of-pocket costs.

In addition, proposed amendments to some health care reform proposals could also reduce patient access to critical cardiac care.

Together, such changes would decrease access to cardiac care, particularly in rural areas, and diminish competition and choice. And they would take lifesaving tools out of the hands of trained physicians.

"The most costly heart attack is the one that could have been prevented," said Dr. Levy. "These proposed changes will only hurt our ability to diagnose and treat patients before a heart attack strikes."

Patient-Focused Solutions
Successful treatment of patients with heart disease requires speed, access to the latest technology and access to specialized expertise. Certain changes now under consideration would cause lengthy delays in diagnosis and treatment, limit patient access to technology and, by forcing some physicians to curtail services, severely limit access to cardiac care. Guarding Hearts Alliance would like Medicare and Congress to take the time necessary to explore thoughtful and proper changes to avoid the unintended consequence of limiting access to quality cardiac care.

Understanding that reform is needed, Guarding Hearts Alliance urges Medicare officials and Congress to consider measures that:
 Support strong credentialing and accreditation programs to make certain that diagnostic equipment is used properly and cost-effectively.  Encourage the use of criteria to help physicians determine which tests are appropriate based on a patient's symptoms and medical history.  Provide for disclosure of physician ownership in hospitals and imaging equipment to promote transparency.  Ensure patient safety through the creation of emergency care provisions and protocols at physician-owned facilities.

DownÂ’s Heart Group today launched itÂ’s campaign to lobby Ministers and MPÂ’s regarding the Governments failure to implement recommendations made in the report of Paediatric and Congenital Cardiac Services Review Group published in November 2002.

In March 2001 the Department of Health set up the Paediatric and Congenital Cardiac Services Review Group (PCCSR) to advise on the implementation of recommendations made in the Kennedy Report, also taking into account the findings of the inquiries at Royal Brompton / Harefield and Alder Hey. They made recommendations to Department of Health Ministers which will ensure high quality, patient-focused effective, efficient, accessible and safe services for children and adolescents with congenital heart disease and their families as part of the NHS “Agenda for Change.”

The Review was assisted in no small part by many organisations and individuals who gave of their time and experience to ensure the resultant recommendations would provide a detailed and valuable framework for the future development of Paediatric Cardiac Services. For many parents this was an highly emotional topic to discuss but their motivation was the realisation that they were best placed to ensure the necessary changes, which would provide the high quality treatment our children deserve.

Speaking at a Conference on December 15th Dr Stephen Ladyman, the Parliamentary Under Secretary dealing with the PCCSR, stated that “There is no part of our population that’s more important than our children. And nothing more important than the quality of care that we provide for our children. That is why services for children and improving the health of children have been given such prominence by the Government.“

Yet despite these comments, over two years after the Report was published, the Government has failed to provide the finance and backing necessary to implement these important recommendations.

DHG Chairman, Cliff Lake states “Down’s Heart Group, along with leading paediatric cardiac specialists, are appalled that findings from the Kennedy and Evans Reports are still being ignored. Despite all the Governments early promises made by Alan Milburn after Bristol and setting up a specialist review, the only constant thing seems to be NO CHANGE!”

About 47% of those born with DownÂ’s Syndrome have heart problems, Down's Heart Group is a national charity (1011413) providing support and information specifically relating to these. "Hotspots are a way for European fixed line operators to extend their broadband networks and for mobile operators to finally profit from their 3G investments," according to Colin Carroll, a senior research analyst for ON World. The biggest revenue opportunity however is for venue owners who will generate additional product oriented sales from their locations. As venue owners become more educated about the benefits, ON World believes there will be an increasing number of cafés, health clubs, hotels, retails stores and malls that will offer hotspot services for free.

A Prime Environment for Hotspots

The European environment is nearly ideal for offering public broadband access due to its concentrated population, high mobile phone penetration, growing broadband adoption, cafe culture and centralized telecom infrastructure. User adoption will start slowly due to high access costs, unavailability of multi-mode cell/wifi phones, limited roaming and because managed network operators are still educating the market at the local level.

ON World believes these limitations are short term and 2004 will see the launch of several multi-mode cell/Wi-Fi phones that will provide network operators with new ways to extend their broadband reach and create compelling new services.

By 2009, ON World predicts that 15 percent of all hotspot users in Europe will be frequent users, defined as those who use hotspots more than four times per month, up from 3 percent in 2003. Frequent users will increasingly opt for “all-you-can-eat” hotspot subscriptions that will likely be part of a bundled service. Remaining hotspot users include a variety of revenue models including an increasing number of users who will access hotspots for free. European hotspot subscription revenues are projected to be $898 million in 2009.

Land Grab Moving Fast

Carroll notes that the land grab phase of the hotspot rollout in Europe is quickly closing and as WiFi devices proliferate, the next phase of WiFi enabled services will soon arrive. The rapid hotspot deployment phase of the next three to five years will be supported and driven by operational service support (OSS) and hotspot access providers such as AirPath, Nomadix and Pronto Networks as well as aggregators such as iPass, GRIC and Trustive.

ON World believes Wi-Fi enabled devices will become widespread by the end of 2007 and this will result in increasing demand for hotspot oriented multimedia services, voice over wireless LAN solutions and seamless roaming solutions by companies such as Appear Networks, BridgePort Networks, Kineto Wireless, Megisto Networks and WeRoam.

As with all things in Europe, there are identifiable differences from country to country. Hotspot numbers, costs, adoption rates and usage figures differ widely, and some of these findings are quite surprising. According to ON World, the leading countries by overall hotspot deployments are the United Kingdom, France, Austria, Switzerland, Germany, Netherlands, Sweden, Italy, Norway, Finland and Spain. Portugal has shown rapid hotspot deployments recently and has almost as many as Spain. Denmark's TDC just announced that they have deployed 400 hotspots at Statoil, McDonald's, and Baresso Coffee locations and plans to have 700 by year-end. Eastern European countries such as Poland, Hungary and the Czech Republic are particularly good opportunities for hotspot oriented developers, network operators, service providers and venue owners due to relatively high mobile phone and broadband adoption and low but increasing hotspot deployments.

Robert D. Putnam is the Peter and Isabel Malkin Professor of Public Policy at Harvard University, where he teaches both undergraduate and graduate courses.
