With baby boomers nearing retirement age, the Medicare hospital insurance fund will face bankruptcy in 14 years, and the Social Security program trust fund will be on the rocks in 38 years. A drop in the population growth rate and rising health costs aggravate the problem.

Analysts estimate the government may have to issue huge amounts of debt to patch up unfunded future liabilities of $43 trillion projected for both programs that could send bond rates up to double-digit numbers.

Analysts express concern about the necessity to cover the liabilities combined with BushÂ’s intention to cut Social Security taxes. His proposals include reducing existing payroll taxes by 2% to fund private accounts, managed by retirees themselves. However, the creation of these accounts will require an estimated $104 billion to $1 trillion, and the funding will come either from the cut in benefits or a tax hike. One more option is to raise the mandatory retirement age.

Some experts suggest that shifting funds from Social Security to private accounts will increase the fees of the Social Services industry without proving better guarantees for employees. A lot has to be done to increase employee participation in retirement plans that now is at 42%.

Internship program allows NTI students to gain valuable on-site experience with heating and air-conditioning/refrigeration while attending school.

Since 2008, Northeast Technical Institute (NTI) has trained and certified nearly 200 HVAC/R students at its Scarborough campus. In just under five months of hands-on training and earning national certifications, many of NTI's graduates are now earning good pay and benefits working with some of the area's top HVAC/R companies including Johnson & Jordan, Miller Refrigeration, Air-Temp, Solaris, AAA Energy and more. With such positive results, it was no surprise that NTI decided to offer its first HVAC/R class to the Bangor community this past November.

NTI offers a variety of training programs at its Bangor campus located on Stillwater Avenue including Medical Assistant, Medical Coding and Billing, Professional Tractor Trailer Driving (CDL A), and more. But HVAC/R training requires a specialized training area because of the training equipment, as well as proper ventilation for heating and refrigeration/air-conditioning units. Therefore, NTI's HVAC/R training facility was built to specifications and is located at the Danforth's Plaza on Route 2 in Hermon. The new training facility allows students to install, repair and operate a variety of HVAC/R systems.

In addition to offering training at its shop, NTI students are taking weekly field trips with NTI training partner E-Conn, Inc. Each week, students are gaining real life knowledge and skills in installation and repair through the internship program with E-Conn, Inc. Students analyze blueprints, install air handlers, gas fired condensing furnaces, gas piping, and complete duct work. David St. Germain, Maintenance Manager at E-Conn said, "We find that NTI students are exceptionally well trained and prepared for their internship. And most importantly, these students are gaining invaluable experience through this internship and it will give them the foundation they need to get a jump-start in their new careers upon graduation at NTI."

Recently, the class completed a field trip in Monroe to install a wood boiler for a home owner. This gave the students a new outlook on hydronic systems with a solid fuel back up. "To have the hands on experience instead of just reading about it and seeing pictures was great" says NTI student, Sean Clegg. He continued to describe this field trip as being very informative for the class and described the building owner as very enthused to be giving the class the opportunity.

Fellow student Terry Dauphine explains this training as "extremely important and to be able to work with customers is a major part of this field. You need to be able to work with small businesses and large corporations". Terry explains that installing these systems allows them to see the every day difficulties that may arise in on-the-job settings.

Top Skin Conditions Facing People of Color

Leading dermatologist describes unique skin challenges

People of African, Asian, Latin and Native American backgrounds know that their skin, hair and nails are subject to conditions that do not affect lighter skinned people. "While most of these are not serious, they may be disturbing, troubling or unsightly," says Joshua Fox, MD, a leading dermatologist, founder of Advanced Dermatology and an official spokesperson for the American Academy of Dermatology and the American Society of Dermatologic Surgery.

"Fortunately, your dermatologist is well acquainted with these conditions and can help people with dark skin to diagnose and treat their concerns early and quickly," says Dr. Fox. He offers the following listing of the top skin conditions faced by people of color and how it differs from lighter-skinned individuals.

1. Acne - Many dark skinned women suffer from acne vulgaris, sometimes combined with hyper pigmentation, or skin darkening in spots or patches, which occurs in response to the outbreak of acne. Unlike fairer-skinned individuals, the discoloration plays an integral role in treatment. Surprisingly, these patients are more bothered by the discoloration than the acne itself. Treatments may include topical skin lightening creams for dark spots, sunscreen and topical, prescription acne medications.

2. Dermatosis Papulosa Nigra - These brown or black raised spots, which commonly affect African American women and people of Asian descent, may look like moles or flattened warts. They are always benign, never lead to skin cancer and are not harmful. However, some people do have them removed for cosmetic reasons. Typically, up to 50 percent of Dermatosis Papulosa Nigra patients have a family history of the condition, and up to 35 percent of adult blacks in the United States have it. They are easily removed with minimal to no marks.

3. Eczema - Very common among those with brown skin, eczema is an itchy, irritating rash that can occur in skin of any type or color. However, according to the National Eczema Society, it is found twice as often in black skin. When it does occur, differences in the structure of black from Caucasian skin can cause related problems including excessive pigmentation and a thickening of the skin that can also cause changes in skin color. "Because eczema is harder to identify in dark skin, and is often confused with psoriasis or fungal infections, getting the right diagnosis and treatment can be difficult," Dr. Fox says. "Once the correct diagnosis has been made, topical medicines are quite helpful."

4. Keloids - Scar tissue caused by abnormal healing of the skin occurs frequently in individuals with brown skin. Any time dark skin is injured, the risk of keloids is dramatically increased. According to Skin & Aging magazine, up to 16% of black and Asian people suffer from keloids. Keloids can develop immediately following an injury or take a long time to grow. Sometimes they itch, are painful and burning or feel tender to the touch. Treatments include cortisone and other injections, radiation therapy, pressure dressings, silicone gel applications and several types of lasers are extremely helpful. Keloids can also be removed via traditional or laser surgery.

5. Traction Alopecia - This hair loss condition is caused by damage to the hair follicle from constant pulling or tension over a long period of time. It is most common in African Americans who braid their hair tightly, but can also be caused by over processing of the hair through dyes, bleaches or straighteners. According to the National Institutes of Health, some 17% of African-American schoolgirls (6-21 years) and more than 30% of women (18-86 years) suffer from traction alopecia. While this condition can be reversed if diagnosed early, it can lead to permanent hair loss if it is undetected for a long period. These people - mainly women - should change to looser, gentler hair styles and should see a dermatologist. Unfortunately, no medical treatment is available to reverse late-stage traction alopecia; hair grafts, transplants or wigs have been identified as the only practical solution.

6. Melanoma -- Those with brown skin often have a false sense of security when it comes to skin cancer. "While increased skin pigmentation offers some protection against the sun's harmful UV rays," Dr. Fox says, "melanoma can and does occur," unlike in the lighter-skinned individual, it occurs especially on the palms, soles, fingers and toes, nails and mucous membranes (such as the mouth or nose). "A new dark mark or a mark that changes in size, shape or color in these areas should be seen by a dermatologist immediately. Even people with dark skin should always use a sunscreen when spending extensive time in the sun," he says.

7. Pigmentation - Women with brown skin, particularly those of African and Latino descent, have a high incidence of hypertension, diabetes and heart disease and thus take medications for those medical problems. These, as well as other commonly used medications (LIST MEDICATIONS THAT CAN CAUSE PROBLEMS) can cause various types of allergic reactions that frequently lead to hyperpigmentation, or dark patches. While the dark spots can fade over time, women should seek treatment early. Procedures such as chemical peels and skin bleaching, designed to lighten the darkened areas, can help, as can diligent use of sunscreen. 8. Vitiligo - Skin gets its color from pigment cells. When those cells are damaged or destroyed, they no longer produce pigment, causing white or colorless spots to appear. The spots can grow larger over time and eventually blend together so that large portions of the skin have no color at all. While no one knows why this occurs, many suspect it is related to problems within the autoimmune system. Vitiligo also may be hereditary. While people of any skin color can develop Vitiligo, it is most obvious and therefore debilitating on dark skin. One common treatment is PUVA -- a repigmentation therapy involving the drug psoralen combined with exposure to UV light. A simpler, newer and equally efficacious treatment is narrowband UVB light treatment. The newest laser treatment though is that of the Excimer laser 308nm which markedly shortens the number to treatments and spares unwanted streaks. This process help increase the amount of pigment cells at the skin's surface. Other treatments include prescription-strength corticosteroid cream, light/laser treatments and, in rare instances, skin grafting.

Uninformed Americans overbuy nearly every type of insurance from overzealous agents who put commissions ahead of clients or insurance companies that deluge them with marketing messages.

Being over-insured places needless financial burden on young families, baby boomers and retirees, says Paul J. Mauro, CLU, CHFC, Managing Partner, Legacy Financial Advisors, Inc. in Milford, MA. MauroÂ’s firm delivers a complete menu of financial and estate planning services. He says, "Integrate insurance with financial and estate planning to avoid the common and costly insurance mistakes consumers make."

Here are some of MauroÂ’s favorites:

Too many property insurance companies--Consolidate homeowner and auto insurance with an umbrella insurance policy. Bundling policies can cut premiums by 15%. If you buy a $1 million liability umbrella insurance plan, do not increase the underlying limits on the home and car above $300,000. This is wasteful.

Buying whole or universal life when young--Young breadwinners with dependents, a big mortgage and more responsibilities than money, should never buy a small permanent, whole-life, universal life or any life insurance with an investment component. This leaves young families grossly underinsured because they cannot afford enough coverage. A 35-year-old father of three with a $50,000 annual income needs a $1 million policy. Level term is $400. A $1 million universal life policy, has cash value, but the same coverage costs about $6,000, hence young families often buy too little death benefits.

Not buying insurance at work--Employers can help you lower insurance cost by sponsoring many types of insurance. For example, buying voluntary excess disability insurance as part of a group at work usually costs a third the price of an individual policy. Other employer-sponsored plans offer good value as well.

Procrastinating on long-term care insurance--Look into employer-sponsored, long-term care coverage too. Read the fine print. Make sure you can take this coverage with you if you leave the company. DonÂ’t delay. Long-term care insurance at age 70 costs $4,000 annually, totaling $60,000 to age 85. The same policy, through an employer group, bought at age 50 costs $1,200 per year or $42,000 to age 85; saving $18,000.

Not evaluating your insurance at age 60--Re-evaluate coverage. Since most disability insurance covers you only until age 65, it may be wise to drop it as you near retirement. A disability policy at age 40, could pay you for 25 years at $40,000 per year or $1 million total. If you are over 63, the policy will only pay out for less than two years -- a total of $80,000 in benefits.

Paying for accident insurance--Get rid of "accidental death only " benefit policies. As you age, death or injury by accident, as opposed to illness, becomes increasingly less likely. "I met a couple with $250,000 of accident-only life insurance on each other. They were 70 and did not snowmobile, horseback ride or skydive."

Not comparison shopping for Medicare Supplemental Coverage--Buy high-quality medical and Medicare supplemental coverage through whatever group arrangement you can find. "A 72-year-old Massachusetts couple was paying $12,000 annually for an individual supplemental policy," says Mauro, adding, "They then joined an HMO and cut their cost to $6,000." To find a group, contact your former employer, Council on Aging or a small-business network if you are self-employed.

Failing to consult with an attorney--"Review your business plans, estate plans, and medical documents with your lawyer. In a recent case, a family was paying thousands for insurance to cover the death tax for their children. After paying a few thousand dollars for legal services, they saved nearly $7,000 annually in premiums, because their estate was now tax free. Likewise, living trusts for both spouses can cut the cost of long-term care insurance in half," Mauro says.

Falling prey to insurance gimmicks--Some insurance is just a bad buy. Individual dental insurance tops the list. Illness-specific coverage is a close second. Collision insurance on cars over seven-years old is dubious. Also, drop most maintenance contracts, lost credit card insurance and creditor insurance. If you bought mortgage insurance (PMI ) when you purchased your home, the equity build-up may allow you to drop it. Mortgage term and credit card term life policies are usually overpriced if you are healthy. Also make sure your credit card covers the rental car deductible and donÂ’t buy extra coverage.

Being a know it all--Do not assume you know everything. New insurance policies have been developed that are big improvements over those available 10 years ago. Find a quality independent agent or planner to help you review your old policies.

Example, "A Legacy client wanted their granddaughter to inherit their $1 million dollar estate. She was the apple of their eye and they were living on $25,000 annually to conserve principal. I asked them if they thought their granddaughter wanted the house and bonds, or a check for $1 million tax-free from an insurance company. All grandpa said was, ‘She’ll want the check.’

"Our client then bought a new joint-life insurance policy with low premiums to endow the granddaughter. This freed up cash from the estate and, as a result, we increased their income to $45,000 annually, while guaranteeing the granddaughterÂ’s inheritance," says Mauro. Joint life policies allow elders to spend some principal to live better while leaving a tax free estate.

Participants earned CEU credits toward the credentialing programs of the AAMM, including the Certified Medical Staff Recruiter designation and the Fellowship in Medical Staff Development. Additionally, the AAMM is fully accredited by the Accreditation Council for Continuing Education to provide continuing medical education for physicians.

Ed McEachern and Tim Sheley taught three classes devoted to the topics “Social Media for Physician Sourcing” and “Research on Understanding the Value of Each Physician.”

“The tremendous growth of social media among the medical community has had a favorable impact on physician recruitment,” mentioned Mr. McEachern. “By connecting with physicians who regularly participate in social media—such as visiting ‘virtual doctor lounges’—recruiters can expand their network of prospective candidates for current practice opportunities,” he added.

Determining the value of a potential or actual hire was the topic that Tim Sheley addressed. Whether a physician is being considered for a locum tenens position or permanent employment, it’s beneficial to know in advance what his or her potential contribution to revenue generation will be, based upon statistical modeling.

“In calculating the return on investment in bringing a doctor on board, it’s helpful to compare the doctor’s potential to impact the financial bottom line with his hourly or annualized average compensation,” Mr. Sheley explained. “Using this data, administrators can get a better feel for what a new doctor is potentially worth to the practice.”

Tony Rico tag-teamed with Ed to present a session on “Locum Tenens: When You Need a Doctor Immediately and Credentialing.”

In pointing out the benefits of using locum tenens doctors, Mr. Rico stressed the importance of using contract providers who are fully credentialed, able to be hospital privileged without delay, and ready to accept a practice engagement.

“Health care employers want to feel comfortable when engaging the services of locum tenens providers. They want to know upfront that the candidates have been carefully sourced and that all of their credentialing documents have been meticulously verified,” Mr. Rico explained.

Research presented at the World Congress on Neck Pain shows that the proper use of well-engineered head restraints dramatically reduces serious neck injuries from automobile accidents. The Congress also launched the release of a seven year study on neck pain which produced surprising findings.

Research presented at the World Congress on Neck Pain shows that the proper use of well-engineered head restraints dramatically reduces serious neck injuries from automobile accidents.

The World Congress brought together health care experts on neck pain, automobile safety engineers, and others to review current best evidence about causes, prevention and clinical care for neck pain related whiplash injuries and other causes.

Neck pain after automobile collisions, commonly referred to as whiplash associated disorders, costs the insurance industry $9 billion annually in claims costs, and is considered an epidemic by many medical and insurance professionals. Yet many whiplash injuries could be minimized or prevented altogether by the proper engineering and/or adjustment and use of head restraints.

Head restraints are not head rests; they are essential safety devices which act to reduce the likelihood of sustaining a whiplash injury. Studies by the Insurance Institute of Highway Safety (IIHS) have found that head restraints in more than 60 percent of car models fall short of current state-of-the art protection from neck injury or whiplash. They found that many head restraints were not high enough, or close enough to the back of an occupant's head, to provide effective protection against neck injury in rear-end crashes. Even when the head restraint is well-engineered, people don't always position them properly.

To reduce or prevent neck injury, the top of the restraint should be positioned even with the top of the head or at least to the top of the ears. The distance from the back of the head to the front of the restraint should be less than 4 inches.

In 2009, new vehicles sold in the U. S. will start meeting more rigorous structural standards for head restraints.

The Congress held January 20 through January 22 at the Hyatt Regency Century Plaza in Los Angeles also launched the release of the seven year study by The Bone and Joint Decade 2000-2010 Task Force on Neck Pain and Its Associated Disorders.

"Neck pain is not a trivial condition for many people," says Task Force President, Dr. Scott Haldeman. "It can be associated with headaches, arm and upper back pain and depression. Whether it arises from sports injuries, car accidents, workplace issues or stress, it can be incapacitating. Understanding the best way to diagnose and manage this problem is of high importance for those who are suffering and for those who manage and pay for its care."

Primary study findings from the Task Force report revealed the following:
Health care providers can apply scientifically validated screening criteria to determine which patients with neck pain after auto accidents will have serious structural damage, and which need only temporary help with symptoms. Once serious underlying conditions have been screened out, imaging tests, such as X-rays, CT scans or MRI scans are typically not needed, and will seldom identify the cause of neck pain. Aging changes of the spine [commonly called degenerative changes], frequently seen on X-ray in middle age adults, are not predictive of neck pain.

For the majority of neck pain the following treatments show evidence of being effective for reducing symptoms:
Exercise Mobilization Manipulation Massage Acupuncture Low level laser therapy Analgesics

Prolonged treatment was not shown to be beneficial. It was recommended that patients do not continue treatment that doesn't provide improvement within a reasonable period of time, and instead seek out alternative treatment options.

The study revealed that the following treatments were unlikely to help in relieving neck pain:
Collars Ultrasound, electrical muscle stimulation Most injection therapies Radio frequency neurotomies

The World Congress on Neck Pain was organized by the Canadian Institute for the Relief of Pain and Disability (CIRPD) and the American College of Occupational and Environmental Medicine in collaboration with the National Institute of Occupational Safety and Health and 38 professional and consumer organizations.
